• *The cumulative and current school quarantine and isolation counts will be updated every  Occasional delays may be expected due to case volume.
• All data is preliminary and represents a snapshot of time.
• All isolated counts include students and staff who were confirmed positive for COVID-19 via test and those individuals who isolated due to COVID-19 symptoms who were not tested.

This form is for schools to report COVID positive staff and students or staff and students who have been in close contact with a COVID positive person. 

This reporting helps TLCHD quickly identify any school-related close contacts who may have been exposed to COVID-19 and proceed with the appropriate next steps.

All Lucas County schools must submit new and cumulative case numbers per the Ohio Director of Health's Order.

An updated version of the excel template must be submitted by email to TLCHD every Monday by 12 noon: ReportCovid@co.lucas.oh.us

Districts may submit the same file each week, adding new and cumulative cases for each reporting period on new rows.
