Death Certificates

Online orders may be submitted here.

An additional fee of $7.00 (plus expedited UPS shipping charges) applies. All major credit cards, including American Express, Discover, MasterCard, and Visa, are accepted.

Please submit the following:

• Birth/Death Record Request Form
• Payment for the certificate
• Self-addressed stamped envelope to Vital Statistics at the Toledo-Lucas County Health Department at 635 N. Erie Street, Toledo, OH 43604.

Upon receipt of your request, your certificate will be mailed back to you within 2 business days. Please allow 7-10 business days for the return mail.

The Office of Vital Statistics is not responsible for delayed or lost mail. If you believe mail is lost or delayed, please contact USPS for assistance with locating your mail before contacting the Vital Statistics office. Replacement copies of certificates reported lost in the mail will be sent by certified mail only after 30 business days. The customer will be responsible for paying certified mail charges.
