Purpose
To assess community health needs and collaboratively plan to improve health outcomes in Lucas County. It serves as a collaborative platform for the Toledo-Lucas County Health Department, hospitals, and other local organizations to fulfill health assessment and improvement plan requirements for the county's residents.
The Healthy Lucas County coalition is a broad partnership of health, government, and community entities working to assess community health needs and implement plans to improve health outcomes in Lucas County.
Established in 1999, the HLC coalition develops and implements a community health assessment (CHA) every three years to inform collaborative strategies aimed at reducing health disparities and improving health status. We’re especially focused on addressing the needs of residents living in low-income areas and helping everyone live to their fullest potential.
Healthy Lucas County is supported by our funding partners. An Executive Committee made up of decision-makers from each partner organization governs Healthy Lucas County, and a list of our member organizations can be found here.
Healthy Lucas County is coordinated by the Toledo-Lucas County Health Department.
Community Health
Healthy Lucas County performs a community health assessment every three years. The latest one, the 2022/2023 Lucas County Community Health Assessment, was released in December 2023. The assessment has data about Lucas County children, youth, and adults. These are the presentation slides from the release event.
